As your school nurse, I look forward to partnering with you to ensure a healthy and safe school experience for your child. The following are FAQ facts:
Physical Forms: Physicals are required by the state of Connecticut for any child in kindergarten, seventh grade and new students entering a Connecticut school system for the first time. These physicals must be current (completed within one year of school entry). All asterisk (*) areas on the blue form are mandatory and must be completed by a physician. The State of Connecticut requires that the original physical form be on file for all students. Faxed copies are not acceptable.
Emergency Contact Numbers: Each parent is asked to complete an emergency contact sheet at the start of the school year or when their child enters the school system. If contact numbers change during the course of the year, please contact us with the change so we can update our files. It is very helpful to have a local contact for your children.
Medications: School policy allows for children to receive Tylenol or Motrin with a parents written permission. These pain relievers will be dispensed as a comfort measure to alleviate the pain of such things as dental issues, sore throats and headaches related to allergies. Children who require prescription medication to be given at school, such as asthma inhalers, must have a doctors order sheet completed. Per State law and school policy, the medication must be brought to school by an adult. The medication must be in an original pharmacy container with the child's name, dose and medication clearly spelled out. No medication, either prescription or nonprescription, can be carried to or from school by the child. Return to top
Illness: School policy states that any child with an oral temperature of 100 degrees must be sent home. Please keep your child home from school for 24 hours after the last episode of vomiting, temperature above 101 degrees, and the start of antibiotic therapy. Return to top
Please contact the health office for absences beyond two days or if a serious infectious illness is diagnosed.
Orthopedic Issues: If students have an injury to a bone that requires crutch use in school, a doctors note is needed to document the injury and restrictions the child may have. Please bring the child to the health office with the doctors note or the emergency room discharge instructions after the injury so each situation can be reviewed individually. A child on crutches can not ride the bus. Return to top
